Question 1-Peer review of one particular answer of 10 randomly selected students.

Answer- I have chosen the following case to review:

A 40 year old male presented to the hospital from yadagirigutta with the chief complaints of irrelevent talking and decreased food intake since 9 days.

My answer and understanding of the above case:

The patient was talking,as well as laughing to himself. He was also unable to lift himself off the bed and move around,and had to be assisted.

All the above things are signs of mental confusion.

He was alcoholic, had stopped drinking .he developed seizures following cessation of alcohol for 24 hrs, which was associated with restlessness, sweating and tremors.

All the above are signs of alcohol withdrawal delirium.

Albumin is decreased,this may indicate liver or renal problems.

Most probable diagnosis would be 

1.Wernicke's encephalopathy secondary to chronic alcohol dependence.

2.Alcohol withdrawal delirium.

   The case presentation is well done with the information of all the required investigation reports.The given ABG analysis,2D echo,USG abdomen clearly emphasis the disease of the patient.The video of the patient reveals complete picturization to analyse the disease.I feel the presentation is very accurate and complete information can be analysed.

History of present illness is presented very well. The images of diagnostic tests are also presented , which make it easier to understand the case.


Hypertensive neuropathy describes clinical syndrome which is characterized by longterm essential hypertension. It develops due to - Glomerular ischemia , Glomerular hypertension and Glomerular hypertension.


Uremic Encephalopathy is an organic brain illness caused by the accumulation of toxins ordinarily removed by the kidneys. It occurs in patients with renal failure when creatinine clearance falls below 15 mL/min. Uremia is the last stage of increasing renal insufficiency, which can lead to multiorgan failure, due to accumulation of protein metabolites. Uremic encephalopathy is one of the main cause of kidney failure.

last month we just began our clinical postings through online.First time it is very difficult for us,but with this blogs we are atleast knowing the basic things like history taking etc.Due do this pandemic our learning became very tough because everything is online. our professors ,pgs ,interns helping us to learn subject through blog.we are unable to attend offline postings in this situation eblogs are very helpful as we are taking up a case.Our HOD of general medicine explaining each and every case.
